Senior Independence Program Case Manager (Full-Time)

Purpose: The Case Manager for the Senior Independence Program Pilot (SIP) will be trained as a Community Health Worker to support neighbors age 65+ so they may be better able to age independently in the home. The Case Manager is responsible for providing eligible TCP services to older adults, assessing for enrollment into Case Management and guiding those enrolled in benefit and service navigation. SIP service delivery will focus on the five Social Determinants of Health: Education, Finances Neighborhood/Environment, Health/Wellness, Social /Community.  Visitation locations include: home, community and businesses.

This is a grant-funded position. Community Health Worker Certification is required and paid for by TCP.

Essential Duties and Responsibilities:

  • Develop and maintain ongoing knowledge of TCP’s basic needs crisis assistance services, processes and protocol
  • Develop and exercise communication skills that convey empathy and build trust
  • Assess client basic needs and motivation/readiness for change for case management enrollment
  • Provide financial assistance per eligibility and guidelines
  • Connect senior clients to community resources related to Social Determinants of Health: economic stability, social and community, education, health care access and neighborhood
  • Evoke a client-directed empowerment goal plan
  • Analyze plan progress and barriers to goal plan
  • Follow – up with clients via phone calls, office and/or home visits
  • Report on outcomes including variances as applicable
  • Follow all case management operational procedures
  • Train and serve as needed in any Programs and Services Department capacity  
  • Actively participate and contribute in all meetings: supervisory, team, department and all staff
  • Assist in the development of Programs and Services policies and procedures.
  • Successful completion of a Community Health Worker Certification is required within 9 months of hire
  • Completion of training on Motivational Interviewing, Emotional Intelligence and Unconscious Bias within three months of hire
  • Willingness to remain flexible if case management support required work on some evenings and Saturdays 
  • Adhere to The Caring Place policies and procedures
  • Other duties as assigned

Qualifications (Required):

  • High school graduate or equivalent required plus four years of social service experience is required
  • Bachelor’s Degree in social work, psychology, counseling, or related field from an accredited four-year college/university preferred
  • Valid driver’s license, clear driving record and proof of liability insurance to conduct home visits
  • Experience working in a multi-cultural, community-based setting
  • Bilingual in English and Spanish (preferred)
  • Excellent oral and written communication abilities
  • Proficient in Microsoft Excel, Word, Outlook and database systems
  • Ability to be creative, flexible, and decisive.
  • Consistent and exemplary positive and calm demeanor
  • Ability to manage multiple, complex, multi-faceted responsibilities simultaneously in a fast-paced environment
  • Exemplary professionalism
